

DOT Physicals - $120
Our Certified Medical Examiner will perform your DOT or CDL physical examination in accordance to FMCSA standards. A DOT physical is a health examination mandated by the FMCSA for commercial motor vehicle (CMV) drivers. A DOT physical helps determine if a driver is physically, mentally, and emotionally fit to operate a CMV.

What to bring to your appointment:

Download your exam form and complete it prior to your visit. Bring it with you on the day of your appointment. Click here for your EXAM FORM.

- Bring a list of your medications.
- Bring your glasses, contacts, or hearing aides.
- If you are diabetic, your most recent A1C results and blood sugar logs for the past 90 days.
- If you have sleep apnea, documentation of CPAP compliance for the past 90 days is REQUIRED.
- If you have cardiac disease, or a history of stroke or seizure, you will need a letter from your cardiologist and/or neurologist.
- Be prepared to provide a urine sample which is required.
- If you are taking a controlled substance (i.e. ADHD medication, pain medication, antidepressants, etc), you must have a letter from your treating physician at the time of your exam. Click here for medication form.
